AI Incidents Are Reaching the Boardroom, but Governance Lags Behind

AI incidents are reaching the C-suite at an accelerating rate, but governance isn't keeping pace. IBM's survey of more than 3,200 senior executives across eight European and Middle Eastern countries finds that 80% of organizations escalated an AI-related incident to their board or C-suite in the past year.
Despite that, only 40% updated their AI governance policies in response. Just 37% say their governance frameworks are built to scale with agentic AI. Most organizations that want to deploy faster than governance allows proceed anyway -- within set risk limits -- rather than pausing to close the gap.
Key Takeaways:
- Accountability gaps are widespread: organizations report unclear lines of responsibility for AI agent failures and divided views on liability.
- Monitoring of third-party AI risk remains limited, even as external AI vendors become more embedded in enterprise workflows.
- Regional pressures differ: EU AI Act deadlines are driving governance investment in Europe, while national AI strategies are the primary lever in Saudi Arabia and the UAE.
